TFC Families Connect

The Families Connect program is a place where our transplant families come together to build a strong supportive community. We meet the first Thursday of every month at Colonial Hills United Methodist Church.

STARS Scholarship &
College Success

TFC offers annual scholarships to reward our transplant STARS. This program is for post transplant young adults aged 12-19. The scholarship applications are due in February and awarded in April.

Pediatric to Adult Care
Transition (PACT) Support

Transitioning from adolescence to adulthood is tough for any young person, but for a chronically ill child who’s lived life in the medical care system, this becomes even more challenging.  Finding care, managing health insurance, setting reminders to take medication, all the while wanting to be normal are all part of the process.The Pediatric to Adult Care Transition (PACT) Support system, in collaboration with the medical team and families, empowers these kids by building key skills necessary to successfully make the transition to adult-based medical care. While we can’t wave a magic wand and make it all go away, we can help decrease the frustration and increase adherence with the lifelong, lifesaving complex medical routines.

Family Emergency &
Co-Pay Assistance

TFC’s Family Emergency Assistance Program offers immediate financial assistance for transplant families who are facing unforeseen and overwhelming expenses. This program assists families with food, utilities, baby care items, medications and more. We even offer temporary lodging for families traveling for medical care.
