The Families Connect program is a place where our transplant families come together to build a strong supportive community. We meet the first Thursday of every month at Colonial Hills United Methodist Church (5247 Vance Jackson Rd., San Antonio, TX 78230). Join us for food, fun, and fellowship.

TFC’s Family Emergency Assistance Program offers immediate financial assistance for transplant families who are facing unforeseen and overwhelming medical expenses. This program assists families with food, utilities, baby care items, and more. We even offer temporary lodging for families traveling to San Antonio for medical care.
If your family is in need of financial or other assistance, please let us know. Call the TFC offices at 210.949.1212 or email us below.
Transitioning from adolescence to adulthood is tough for any kid, but for a chronically ill child with an organ transplant, who’s lived life in the medical care system, this becomes an even more critical – matter of life or death – transition. Finding care, worrying about rejection, setting reminders to take mandatory medication, all the while wanting to be normal are all part of the process.
The Pediatric to Adult Care Transition (PACT) Support system, in collaboration with the medical team and families, empowers these kids by building key skills necessary to successfully make the transition to adult-based medical care. While we can’t wave a magic wand and make it all go away, we can help decrease the frustration and increase compliance with the lifelong, lifesaving complex medical routines.
TFC offers annual scholarships to reward our transplant STARS. The scholarships applications are due in February and awarded in April.
Contact the TFC offices for more information and to get an application.
